Change of Address

Many times when you are changing addresses it could be associated with either a marriage, divorce, or a circumstance in general that requires you to also change your name with several organizations.

We wanted to put together a quick list of resources to assist you with that process as well:

Name Change Checklist

• • Generally most organizations will require proof of a legal name change so you should make sure you have a copy of your marriage certificate, divorce decree, or other legal document ready before proceeding.
• • First you will probably want to correct your Social Security card as many other organizations (including employers) will require a copy of your SSN card. The form you need to complete is called an SS-5 Form and is located here.
• • The Internal Revenue Service (IRS) is always a concern for people but they actually will get notified automatically by the Social Security Administration so no additional work is required for them.
• • You also need to get your driver’s license updated with your State Department of Motor Vehicles. You can follow the links on our DMV pages to take care of that (perhaps at the same time you change your address on your license).
• • You also need to update your passport. If your passport was issued before 2009, then you need to complete a DS-82 Form if it was issued after 2009 then you need to complete a DS-5504 Form. If you have never had a passport you need to complete a DS-11 Form.
• • You also need to remember to update your voter’s registration information or you won’t be able to vote in the next election.

Of course there will be several individuals that you will want to contact and we highly recommend notifying the same people you would notify for a change of address.
